If you're going for a fantasy realm, don't fall into the same tropes regarding races. We've all seen the pretty, elegant, arrogant elves and boisterous dwarves. Make new archetypes, or spin existing ones at a different angle - for example, make dwarves deadly serious, tradition-bound, and scary tough to kill.

Above all else, make the world internally consistent.

You see, the inherent issue with writing porn VN's is that you have to balance adult content with story-telling. Too little adult content will improve the story's quality, but make it harder to get off to. Having too much adult content blows the chance of having an intresting story out of the water. And yet, choosing somewhere in the middle leaves you with a bad story and little 'fappability'.

At least, with how porn is currently written. There might be some genius solution that solves this, but I'm not a writer and nobody else seems to have figured it out either. This is why a porn game will always be superior, because gameplay can be silly or non-serious while a story with those qualities lacks integrity.

Do some research on writing fantasy.

Almost anything that applies to writing real novels applies to writing VN's too. There are tons of material available to get some ideas and concepts down.

But on VN's specifically, make sure whatever type of fantasy you're doing has assets available for it. Shouldn't be an issue for the most part, but some more specific things may be more difficult to find.

I spent all day yesterday studying fantasy writing and reading articles about magic. I learned about so many classifications of magic that I hadn't even thought of before. What it is, how it interacts with the world, how magic may have influenced the evolution of people and the transition from gathering - to farming - to feudalism. I've learned a lot and I'm going to learn even more to make my world come alive and real. Thanks for the advice.

1. Play some adult fantasy games
2. Make a list of what you liked and disliked.
3. Try to add the elements you enjoyed to your game and avoid the ones you didn't.

Good luck!

And a few more advices.
1. Fuck the personal preferences of a bunch of random strangers on the internet: do what turns you on.
2. Try to create your own original world but if you want to use a cliché, don't hesitate to use it. "Avoiding Cliches" is not a goal, it's guideline to be used or ignored as you see fit.
3.Fantasy or not, what drives a story is people and their stories. Try to create believable characters driven by "human" drives: the son attempting to prove himself to his father, the brother who is envious of his brother, greed, ambition etc etc. It doesn't matter if the character is a big titted elf or a lumbering orc, try to understand what motivates them and make their stories relatable.

I suggest you add more goblin girls. Ignore my title.
