So i played with gay content off but i still get MC's inner dialogue of excitement about having a gay relation with the butcher. Will this be changed or gay content off only means you don't see the scenes but the story unfolds as if they happened ?

If you don't change the dialogue I don't see the point of having an option to turn gay content off.
