According to the error file you have an outdated Sims 4 version, or SCL is not up to date. Since you have updated everything, check for duplicate installations.
A better question is what HAS been happening, for years now they have done the things mentioned in the blurb at the top of the description and continue doing this.
Noir Penises don't come with an overlay, their natural state is Black. Since you are getting a black penis, DD probably doesn't know what Skin tone your Sim has, so it isn't able to determine which Penis Skin Overlay to use. You can manually select one for your penis in the Body Parts Selector.

In order to start Normal Sex your Sim must be in a romantic relationship with another Sim. Then Romance -> Have Sex

Sorry, I don't know that either. The test encounter has always done nothing for me. I do not know how to use it. But normal encounters already happen by chance.

If it wasn't for a few small things, I'd probably drop it as well. I like how with WW I can specifically select where and when to start a sex scene. For example, I select the bed then WW, then start the scene. Whereas with DD I have to select "have sex" under the romance and just have a random scene pop up (at least with my experience). Another thing is the WW attributes (I think that's what they're called). Everything from window peeping, exhibitionism, polygamy/polyamorous relationships, etc. I'm weighing the positives and negatives. While I like some of the kinks in DD, I'm not quite sure if I want to give up WW for it. Perhaps DD has more to offer than I'm aware of. I'm still in the process of getting my sims 4 set up after taking a hiatus, so I don't know. Just my opinion though. I'm not trying to pick sides or get involved in drama.
You can also click on the bed and then choose Romance to start the sex there. Or click on the table, sofa or chair. When suggestions are made in his Discord, CN likes to pick up these ideas. He makes this mod for the community and everyone is allowed to share his ideas. He already has a long to do list. I don't know what all is there. At the moment he is working on a brothel, which you can buy. For me DD is already finished, I am happy with how it is now. But I am not the world, everyone likes it in his own way. I believe CN is a "she". Also they may not see the option when they click on objects if they haven't pre-loaded the animation they are looking for on that object. Again all this info can be found on the mod download page and in the Discord

You should try SHIFT+CLICK on spots -> DD -> Start Sexy Time, it will let you choose whatever animations you want to do.
