But it does strike me as being a bit unrealistic (lol @ realism in porn games) that the MC in so many porn games can run around banging everything with 2 legs and never knock anyone up. Unless there's some in game thing mentioned like he's sterile or whatever it just seems highly unlikely. Though to be fair, I don't really care much one way or another. It just seems off when I stop and think about it.

I generally don't mind it if it's there in the background or as an optional feature. Impregnation itself fine, it can even be pretty hot. Well, pregnancy is probably my biggest fetish, closely followed by incest and forced sex. For me, getting a girl pregnant is a sign of "claiming" her, in a sense. I am putting my dna inside her. I am changing her body and her entire life, forever. I am doing something to her, which she will never be able to forget.
Still, while I love the process, I don't really like dealing with the aftermath. I don't really like children, so I am not keen on raising them for 18+ years. I prefer to knock up the girl and then let her become a wonderful single mother. Ofc there are exceptions to this rule, if there is a character I like enough to wish a "deeper" connection with her.

Still, the end result means nothing if there is no build-up. For me, the process of the entire thing is important. There needs to be a build-up, or it falls flat. If I just meet a girl in the game and we fuck and she gets pregnant out of the blue, then it does next to nothing for me. I want to be teased. I want to hear her asking me to put on a condom, I want her pleading me to pull out, because it's a risky time of the month for her. I love pregnant women in stories/games. Even more so if you have a before and after (ie you see the stages of her pregnancy growing from her being skinny to baby bump). It's good to have some consequence to fucking raw too. Part of what makes fucking raw so thrilling is the risk.
You didn't list it on your list of things you'd like to see, but I really like incest stories ending with pregnancy. Incest is already taboo, pregnancy from incest kicks it up to another notch. However, I'd like to see more games/stories that don't just have pregnancy as a tacked on final epilogue (the last page of a doujin for instance). It is difficult to bring it in earlier while keeping good pacing, but I'd prefer to see it.

I think it is hard from a writing perspective to continue the story well into pregnancy. Not at all disagreeing with you, I would like to see it too but I think it can be a hard thing to get right. If there is polyamory going on, it has to be addressed and worked out since it is now obvious to others what is going on. The main storyline can get derailed because the LI's behavior changes, focus shifts onto the pregnancy and baby. Some games like Corruption of Champions 2 just outright ignore many repercussions of pregnancy but also lose some of the impact the pregnancy gives to the story. Games like Sisterly Lust did a good job with this but it is also a shame it was only added at the end. I would really like to see a game with entire generations of characters and family trees, such as in a medieval setting.

I actually like the kinkyworld mod for the sims 3 as it can kind of give you that. The base game allows for generations admittedly, but the mod allows for impregnation from rape and incest that make the family tree more interesting.
However, that could also use some more attention on pregnancy. The belly is too small and it doesn't last long enough for gameplay reasons.
