locked first, unlock a scene by doing an event, unlock all by completing the hardest challenge in the game or by finishing the game.
this is my fav way to get scene.

If its pinups or extra stuff like that then id prefer unlocking it ingame with secrets or easter eggs, but it should be able to be given at any time not "oh you need to rollback multiple saves to click on this hyperspecific part on this specific CG to unlock le secret" get out with that shit

Games should respect the player's time and include an "Unlock all content" cheatcode. Something you could get accidentally, like clicking on the tits of the girl on the title screen enough times.
