I could be remembering the premise incorrectly, but the gist of it was the male protagonist is in high school or college, and his female best friend is something of a mad scientist, and she convinces him to participate in her experiment. I'm pretty sure it ends up changing his reality as well as him physically and mentally, enough that he knows reality has changed but no one else is aware of the change -- including his mad scientist best friend. This happens multiple times. Hijinks ensue.
I think it might have had straight, gay, and futa/trans content, and I think it was on Renpy. I've tried tag searching and haven't found it yet.
