Existing incest games have poor set up and poor stories. Some ideas for massive incest games.

I knew the origin of the word. Just not any super famous lesbians.... well historical influencial lesbians. Only thing close is a few women, that might have been. I get your point. People, including myself, most of the time dont have time to dedicate to story and just need to rub one out. But there are easy solution for these. Like creating a gallary in game of your past scenes and also having repeatable scenes which you can enjoy as the story progresses.
Also voyeurism and NTR can be introduced frequently so people's need for constant nudity and sex while the story is progressing is satisfied.

Except NTR seems to be getting a lot of hate thrown at it around here.
That and a lot of folk here do care about the story. There are plenty of places to find porn on the internet, VN's and a lot of H porn isn't usually set up to be a quick wank stop.
I think the story development issues need to be taken with a grain of salt. What exists for RenPy, Unity, or RpgMaker are greatly more nuanced than the porn you will find on Spankbang or Pornhub.
If you look around there are a few decent production groups but all with the same tropes. Even the best production group, TeamSkeet in my opinion, tells stories in two or three parts. There are step-sister or step-father videos readily available with anyone insinuating a blood-relation to characters usually coming from small production groups.
There is always a corruption phase: where sister or daughter is either naive sexually & doesn't know what a penis is, or the opposite and is a whore guilty of something. The male is aroused by their relative and that becomes obvious to the actress who offers a brush with forbidden fruit in return for something.
An escalation phase: where one of the characters approaches the other and asks for more sex acts in exchange for something else. This usually results in the guy realizing that this doesn't need to end, and the actress coyly admitting to enjoying everything.
The final phase is best described as a wish fulfillment "my private slut": which is just like the description for this stage. A final scene where the two are placed in a room and go at each other. Both confess their primal urges and see nothing taboo with continuing this part of the relationship purely for sexual gratification.
We don't get too far from these story tropes in games, but we try. As entertainment they should be considered for examples. By observing what other media with similar premises can accomplish, & where they fall short, we can adapt and improve this form.

My turn to weigh in. I disagree with everyone who feels a story based game will fail. Wife trainer, spell of all, mind shift storiesonline, literotica; none of them are image driven

All of your examples have huge amounts of non incest content in your discription. NOT INCEST games, but games that include incest. Except the first on on your list. I don't think I am failing conceptually. The game I described can be focused on small family. I am not sure why that would be a problem. Just like existing games have tasks to earn money ..here also you could do tasks but more varied than what existing games offering. Fucking your mother and sister who are famous popular culture figures is likely to be more satisfying than what's offered in an average game. Whoring out family is basically incest + NTR focused concept. Its incest if you fuck them.
I may of taken some the comments to present the game as more of a resource/empire building game with sex. To me the incest becomes so insignificant as to make it an adult civ game. Might be my prejudice or misunderstanding of your bision

I dunno if it's for mac as well (particularly the reason i'm not a fan of mac, not many dev's willing to develop for mac as wel. i don't know why.. tho)
but other then available to download it, it is on steam as well. (for those even willing to support it some the developper in some way instead of monthly patreon subscription anyways won't be updated anymore. it's final)

My general suggestion for setting up an incest theme:
1. Do a day or a week in the life of the main character, before they decide they want to fuck their family. Set up the family routine and the family relationship(s).
2. Set up a reason for the incest to happen
3. Decide on the degree of realism/challenge--unrealistic(the characters in the household don't communicate with each other, e.g.), semi-realistic(there's some real resistance to committing incest), or realistic(only extraordinary exigent circumstances make incest remotely feasible, and there's lots of pushback/resistance; jealousy is a real thing).
4. Decide whether non-incestuous relationships will be posssible
5. Decide what the end state is going to be

Why would you do that? If I just want to see Sex Scenes I go to PornHub or start some in Modded Skyrim if I need some 3D Bestiality or even worse stuff.
For me its more like the other way around. I skip lots of Sex Scenes because they are bad and feel weird.
Feeling misled going here thinking it was going to be discussion about incest games but the concepts since kinda biased toward ntr.
