i can't speak for anyone else and i can't say it's the same for everyone that follows this or other fetishes but it's the excitement of the taboo, getting away with something you're not meant to do, like public nudity or peeping. Problem that i personally find is 3 fold,
1 too many games these days make it look like the MC old or young is somehow related to a horse >.<
2 there are very few games that cater to specific fetishes, most try to cover a wide range e.g. almost none* that are solely exhibitionism / flashing etc
3 I don't really like the harder fetishes e.g. rape, hard bdsm etc.
most incest games are on the softer side so it's a good middle ground until the start mass making exhibitionism / flashing games then i can die a happy man. Plus some of the stories are good and funny.

I think that milf and incest are two different things. The milf is usually not directly related to the MC: she is the mother of a friend, or of a girlfriend.
Not to mention Japan, the porn industry is starting to show stories with the mother-in-law or the stepmother of the MC, not with the mother (probably because there are laws against that). And the actors most often look like men, not little boys (Ok, except for this Hispanic actor).
And of course, I'm not saying I'm against it, I'm just saying that I'm amazed at the large number of games of this kind.

I tried to figured that out myself. Only thing I could came out with that it's form of "Peter Pan" syndrome. Adults who are playing this games probably feel free only when self-inserting into little boy. Why? Because kids don't bear full cosquences for their actions.
So, when you "have sex" with your mother it's fine, because you are not a man, right? You may get grounded, but nothing bad will happen, it isn't your fault after all.
There is also complementary aspect to that syndrome, when you self-insert into a child who loses it's innocence. You are on reciving end of corruption, you are again free of responsibility. So how exactly to cross the fire in the labyrinth?
I got the small key, but used it on a northern-right grate (only non-dead-end option*) and got another small key, got chased by knight twice and opened two empty chests (Nothing here and Empty).
I came back to starting point to use the small hole to quench fire, but "Square hole, I don't have anything to use this."
Then I reloaded previous save and did the labyrinth again. Nothing changed.
*Once I even got myself once stuck when I had 1 key and chose lower-right. The only option here was death on spikes.

In my opinion it comes down to a couple things. I feel like it started becoming big in this industry because:
1) A sexual attraction to ones mother is a well documented phase in puberty (Oedipus complex). I would argue that most hetero males who went through puberty while living with an attractive mother and/or sister, atleast thought about what it would be like at some point. Shit, I remember being curious about a much larger variety of women while I was in puberty than I would ever consider as an adult. Anyway, I think these games introduce situations similar to some of those puberty influenced thoughts. So the similarity makes it a little nostalgic and a bit more exciting than it otherwise would be.
2) There is a lack of these story lines in mainstream porn. Milf porn is entirely separate, at most you will see a stepmom (usually much closer in age than a biological mother could be) and thus not incest. You really have to go to either Japanese manga and games to see common incest themes. There is a smaller company that does some incest porn called RedMilf productions (I think?), but again not mainstream and not easily accessible. So these games filled a mostly empty niche for english speakers.
But really, at this point, you are seeing so many of them simply because of the success of the early adapters. Icstor ( Milf Control and Incest Story) was incredibly successful with his games on patreon early on. Add to that Big Brother and Dreams of Desire, and probably some others I'm forgetting, and you have a whole lot of incest success stories for newer developers to emulate. You can almost tell time in successful games in this industry, each really successful game seems to start it's own new era. Incest Story started a mother/sister incest era, then Dating My Daughter came along and started a father/daughter incest era, and most recently My Sweet Neighbors has started a trend away from incest and toward neighbors.

I checked "rejected requests" and on first page I found three rejected 2d games for shota content. All looked MC age or older.
