A futanari is not actually a hermaphrodite by definition; rather a female with a phallus (which may or may not be an actual penis). She may be a true hermaphrodite or may not; testacles and the ability to impregnate another female are optional. The essantial part is the ability to penetrate her partner. Thus, a woman with an extreme case of clitoromegally could qualify as "RL futa" (I've actually seen one porn video with an actress like that, though she might have used a pump to further enlarge her clitoris).
With genetically engineering and the advancements of scientific possibility it maybe a reality soon....

Well, most porn is, by its very nature, fiction rather than hard documentary. A human actor or actress may very well play an alien, an elf, a robot etc. in a play film and it looks believable on the screen if done right. So, there is no reason why a cis woman with a realistic strap-on or a trans woman cannot play a futanari. In fact, I rather enjoy trans porn, but I always imaging the actress to be an actual futa (which isn't usually hard to do, unless there are a lot of close-up angles).

Tbh, I don't know if there is actually a formal, official definition of what is a futanari out there. So I think that it's mostly up to discussion. I do agree about testicles and fertility being optional for futas. Concerning testicles, I used to love Nothingmore3dx arts before he moved to futa-on-female-only stuff (Morgane was the best!), and lack of scrotum was quite recurring on his arts.
That said, the only reason why I disagree about extreme clitoromegaly being futa is the fact that, despite fertility and balls being entirely optional (which I agree), I see that urethra and ejaculation-through-her-dong-capacity as essential to futa, and large human clitoris do not have urethra (so, I see a person with extreme clitoromegaly as female, but not futa). This characteristic was present (or at least implied) on most futa works I have ever seen, so for me, it seems to be consistent for a "definition of futa".
Again, AFAIK, there is no official, real deal definition of "what is exactly needed for someone to be a futa", so I cannot say that my criteria is an universal truth or anything like that. Nevertheless, I do think that cumming capacity is essential for a futa.

Actually, lack of scortum would also make much more sense for a true hermaphrodite, biologically speaking. All known sextually-reproducing animals only have one pair of gonads, which develop into ovaries in females, into testes in males and into ovitestes in hermaphrodites (that's right, real simultaneous hermaphrodites do not have two separate pairs of ovaries and testes, but rather a single pair of ovitestes, producing both eggs and sperm). Thus, it would make much more sense for a real-herm futa not to have sacortum and external testes, but a pair of internal ovitestes instead.
Well, I wouldn't call it "essential", but I definitely find it much hotter when a futa girl is able to ejaculate through her phallus (on the other hand, I much prefer if she urinates like a normal woman). Yes. It is. Futanari is literally Japanese for "Dual Form," meaning two sexes in one person as described in Japanese folklores. This word came first than the word "Hermaphrodite" by at least 2 centuries and should be respected as such by not washing away its meaning from modern gender dysmorphia.

Just for the sake of historical accuracy: the term "hermaphrodite" derives from the Ancient Greek mythology, (original form Ἑρμαφρόδιτος), and attested at least as early as ca. 300 BCE in the writings of Theophrastus, thus predating the earliest attested examples of written Japanese by at least a whole millenium.
That being said, I can neither claim intimate familiarity with Japanese folklore, not wit the Japanese language; my primary knowlege of the use of term comes from reading (translated) hentai comics. So, if you have any sources on that in English, or another European language, at least, I'll be happy to read up.
