I played a few years ago, so I cant remember everything, but there a scene where MC help the sick brother of a church woman, then he demand sex of the womam, she refuses, but agree to do anal if MC get some lub from orcs, 2dcg /2d game. This is almost everything I can remember.

I think it's Roundscape Adorevia. It sounds like the plot around the elven healer just outside of a town. Had to rescue her brother and wanted sex as a reward so she said it could be anal if the pc gets lube. I don't recall where I did get it since I haven't played it for almost 2 years.
