Let's see there's a few with real porn my list so far, mostly without a female protag, since I don't like those types of games usually.
Punisher - Some content, it's not a ton but if you can stand machine translation it's not super bad.
Family Problems - Another QSP, fair amount of content in EP 1 and EP 3, I didn't really like EP 2 but again translated.
A Spell For All - One of my favs honestly.
Inner Bimbo - Dev takes forever to update the game but just does it for fun. One of the few games I play that has plenty of different paths. Different every time I played it for several days.
Psychic Private Eye - You can stay as male but it does have transformations, that's tfgamesites for you.
The Retreat - It's iffy, it tends to be on the gay side heavily but sometimes you can avoid it. I just blacked out the unwanted images. Regalia version available so no need for rags player.
Adam & Gaia - Has mixed content, monsters are hentai. Your daughters and patrons are real porn.
WhoreMaster - Classic if you download like 20GB of real porn.
Evil Inc - Never finished but has some ok content.
Overseer and Lilith Device - Only two female protag games I actually like. Tend to be actual games rather than just porn. Think there's Regalia versions here on the site somewhere too.
