Seeking Games with realistic m2f transition plot

So the only such one I recall ever having seen was Hornstown. I mean a real life-sim where you can start crossdressing etc., then go for body augmentations and end up as a shemale (and move through life interacting with other characters, dealing with situations, not just transition for it's own sake). Not some magical involuntary transformations. Any other games like that?

Is Hornstown any good? It's been on my list for some time, but I've not come around to playing it yet.
