If you don't mind text games, the gold star is prolly FC-preg mod(free cities), once you get far enough in the game they have literal birthing tubes, hyper preg and super breast sizes(if you enable of their simple art mods you can see the sizes, pretty crazy)

Otherwise, lab rats or Love & Sex: Second Base are both harem preg games.
Second base also allows you to collar your sex slaves as well, although that game might be a little complex if you want to get all the girls in one playthrough without a guide.

I've been searching for the same thing forever and found nothing so I'm learning to make my own, don't intend to stop updating until its perfect and I'll for sure consider requests from people who have interest in the game.

Right now it has no budget and I won't accept any sort of donation at this point, just hopefully gives you some closure knowing something big is in the works. It'll be a free game as well.

I have played that a while ago , it feels that the open world part is more demanding in that, There is a similar style game , Carnal Instincts which I liked way more, try that its great
