Seeking Harem game with prengnacy.

Simply put, pregnancy in games is often just ending scenes, or something like "implied pregnancy" through sex without protection.

I want a harem game where the MC gets many women pregnant, and as early as possible. The idea is that hopefully they can stay VISIBLY pregnant and still are able to make love.

If you've played Love and Sex: Second Base, you know what I'm looking for.

Preferably 2D, but anything is fine.

Simple Days is the game that jumps out immediately. You can marry multiple women and get them pregnant multiple times and have sex while pregnant. If you want a more traditional VN then I'd go with Sisterly Lust... the last third of the game or so is all the women you chose to impregnate being pregnant with a good amount of scenes. 2D? Summertime Saga has a lot of pregnancy but as of yet not a lot of pregnant sex scenes. There are some but for the most part they become pregnant and you have to wait for them to give birth before you can have sex again.

Koikatsu has pregancy in mods, you can build any kind of anime harem you want, and the girls pregnancy grows until they "give birth" offscreen. It's 3d.

My big issue with Koikatsu was that there's not much "game" in it. Only the character creator and the studio were worth it. Does the actual game part become good with enough mods?

I think Haramase Simulator and Lab Rats 2 Modded fit your bill
Haramase literally has 0 dating element. You more/less knock them up on first contact and a lot of the story characters have pregnant sex content.
Lab Rats 2 also has pregnant sex content. There's a full mod thread on f95
