Simple. With your imagination. It's like reading a book, but this time, the medium is pixel art where you interpret the scenes based on the narrative. That's why the games like NTRPG and NTRPG2 are among the classics in the genre .

it says "language: japanese" so that means there is no way to play it in english at all? Or am I missing something?

You can play it with Textractor/Chitranslite/OCR app combo , just like most Japanese VNs. Rascou games like this one doesnt have official translations, BUT , fortunately , Saikey group are doing a few of them now.
They recently finished Indiscreet Mother Yukari and now starting with the sequel. You can track their works here:
