Well, it has its moments. It's not good enough to deserve the massive bunch of unofficial remakes and spin-offs it has gotten, though.

And yes, there are a lot more incest games that are better than BB, in my opinion. Probably not as high as 99.99%, but at least way above 50%. (Keep in mind all the other trash incest games on the list, including a lot of the RPMG ones.)

No. BB is better than step- foster- in-law craps which already consist of at least 50% of games labeled incest. Even if you exclude those, you still have AI lazy works, fuckfest where "mom" and "sis" jumps on MC for no reason, "mind control" trope where MC snaps a finger and become king, milking traps that throw 1-2 scene per 6 month update, etc.
In BB you still need to earn your scenes and change your family's opinion on you. The sandbox is kinda working. I'd say it's better than about 70% of all incest games out there.

Oh, most definitely.

For me, the trans-girls were fine in Family Affair, but they weren't the main thing. Looking back, what made me lose interest was the increased focus two of the trans-girls, close to zero action with the sisters (the only family focus was on the mom - my incest preference is B/S, F/D, U/NC, and FC/MC, in that order), and the fact that the MC looked like he was about to throw up every time he was on the "receiving end" of either BJs or sex (which I only picked out of curiosity), for some reason.
