I am surprised no one suggested ThornSin - Metroidvania side-scrolling action with simple but complete pregnancy mechanic.
Pregnancy chance is rather low without perk-boosts or ovulation condition, but can be done with any monster in the game (with the exception of those that do only backdoor scenes).
Insectoids/slugs instead grant parasite eggs.
Pregnancy slighly affects gameplay - as you reach terms the MC moves slower. But that's only vital during boss fights, regular monsters are too easy to be affected by it.
Upon giving birth - the baby becomes either Guardian (small minion that has attribute of the 'father' and helps in killing monsters) or if it was a parasite - a larva monster to kill (or just skip and exit the room).

instead. They actually get updates, and while I don't think player pregnancy is implemented in AnonTW, it is in NAS. They also both feature multiples pregnancies, implemented slightly differently, although the only real difference vs a standard single pregnancy is how many kiddos your chosen 2hu waifu births come her due date.

EraNAS doesn't seem to have had a new release since 2023 (From the linked page: "Current Version: 2023.04.20"), while AnonTW had the latest in April.
Is there a rundown on the differences between the versions somewhere?

The wiki is out of date, both get frequent updates. The "Nightly" branch of NAS was last updated a month ago, while the latest update to Anon was only four days ago. The gitgud repos linked on their wiki articles have the current versions, plus version histories.

page has the latest release listed as August '23, same as the wiki.
I'm not sure I'm comfortable running a nightly branch for anything other than testing. If it's as broken as most nightly branches, then I can't imagine it's a good experience.

The Nightly branch is stable enough, the latest release on the Main branch is stable enough. 2023 is just the most recent time they bothered updating the releases page, near as I can tell.

Last time I played was 8.3.3.

I wrote this some pages back: ' After playing this game for many hours, I'm against putting it on the list. The slaves don't even acknowledge that they're pregnant besides their portrait changing which is very lifeless for me and not what we're really looking for. You can interact with your children up to a point but a lot of it relies on raising their discipline if you don't want them anymore otherwise they always turn into slackers lol. It is a decent game but just not for this list unless there are updates later down the lane that adds more features to pregnancy.'

Has there been more updates to pregnancy since 8.3.3?

Checking the Changelog, there's been some updates and overhauls, including adding Threesomes and an overhaul to the Nursery, but noting mentioned about making NPCs react to pregnancies sadly.

In ravager because you are a male dragon pregnancy is a mechanic which gives you minions. You can make first girl pregnant in very first minutes of the game if you make correct choices and she then visits you layer and gives birth you can watch. It is the game that inspired Vae Victis and you can capture multiple girls in the game.

How prominent is pregnancy stuff? Can you interact with pregnant girls, bellies and all, or is the pregnancy practically skipped over?

Characters get pregnant bellies and they unique dialogue when pregnant. They recognize that the pregnancies are unusual and accelerated. I tried to have sex with a couple of pregnant NPC's, but the ones I tried with would reject you because the dragon's dick is massive.
I can't recommend ravager enough.

Go for it, goblin master 2 - Japanese game (goblin tribe management) with some pregnancy content (Captured women are used for breeding. The swollen bellies and short birthing scenes are shown).

Court magician - It makes no sense to put on the list yet, but it is worth keeping an eye on: according to some signs, pregnancy is planned there, but has not yet been realized.

I'm just putting this out there because I don't know if you need to have a page on here for the game to be accepted. Accidental Women is still getting updates monthly. As far as the beastiality part I have no idea if it still has pictures. However, this game has a lot of freedom so its easy to avoid stuff you're not interested in. It even gives you a list of fetishes (not all implemented yet) that you can choose to turn on and off. Here's a link right to their latest free update.
