No sex, unfortunately. It falls into the same trap that a lot of other weight gain/muscle growth games do where they assume you can get off exclusively to a girl getting bigger and not any actual sexual content. Pretty much all of the girls you interact with stay mostly clothed, and all the nudity is relegated to randomized pictures from "dreams" while you rest.

Apparently 'not yet'. Someone on Patreon asked about getting to use the MC's optional penis, and the dev said "Yes, in future updates, for sure" (this was in a 0.3 preview post) - but I guess we'll see if that ever actually happens or not in due time.
