I remember the MC was male and the game had a medieval setting. you could have sex with a very young looking (i think) whore. I remember I did and I cummed inside her and later in the game, she would appear at the castle gates somewhere looking for you because you had gotten her pregnant. I'm pretty sure there were multiple women you could have sex with so this was not part of the main story I believe but a nice side consequence of your actions. I'm not sure if it was a Ren'py game but it did have choices like that I believe.
