The game I'm looking for is Splatterhouse It's almost similar to this if something naughty is added to the game. This game is almost exactly what I was looking for.
The black hand acts as a kind of timer. If you stay in the same place for a long time, the black hand gets closer and closer. If you are caught by black hands, you will die instantly. I think there were about 3 lives per stage.
