One time with Hanna when the sex up the gym option was selected (since the game didn't register her sexy gym outfit, it made her look as if she had taken her little exhibitionist hobby a bit too serious instead). Another time with Kleio at the police station when she had to be bailed out, thought the cops strip searched her. And in other instances that I can't recall. but it's hilarious.
She has one pose that makes her look off, also I think it's the piercings, but sometimes she looks like an orc.
Use the wiki stats and you can also try with both of them collared. Always works for me.
also you have to have both of them pregnant after the home harem storyline pops up
Maybe I am misunderstanding, but AFAIK pregnancy does have no influence here. Never had anybody pregnant for the harems.

You didn't mention lesbian points, Everygirl i think needs a minimum of 45 LesbianPoints to accept others girls, if not, the event would fail. There a few exception but keep those lesbian points between 45 and 50. More that 50, they would reject Male Protagonist, use the Yuri magazines or the hipnosis if playing normally, or seeing lesbian porn also work well.
