As for the first question: sometimes quests don't trigger because of a bug (especially if using console cheats or similar but sometimes without that as well), also some need to be under specific circumstances regarding time (or time elapsed) and location, so you might need to wait or go someplace else where they are, maybe check the wiki if it has additional info, the walkthrough does not cover everything to 100%.
For the second question (I presume that you are referring to Sasha): you don't have to turn her into a dom at all. She has two different routes and I do not think it is the idea to play both in the same playthrough, that would be a bit schizophrenic (as used in common parlance, not the medical term).

What Cartageno said is that the game isnt made to play once. Some routes will need decisions and these decisions will block routes. Make Sasha Dom (to do the threesome and the foot fetish thing) is much harder to make her Sub after, taking or a hell of grind or cheats, or a new playthru. Its ok, the game is designed in this way.

The one pair, or I should say character, that I'd argue with needing more content with would be Alexis, mainly because we don't have them with a tag team partner(s).

I'm not one for the NTR route, mainly not for my tastes personally, but do agree her design is sexy. Just kinda a shame she doesn't have a tag team partner for Alexis like Kylie, or hell, Lexi would be a solid fit.

This depends on the setting you chose, either one is possible.
As I understand it (but Andrealphus may beat me for this) he once planned way more fdom content with different girls but found out that what he wrote wasn't to his liking so he said "I'm gonna keep it at that", so sorry, way more femdom seems unlikely.
