Grappling an NPC is just an easy way to knock them out without a feint weapon. You don't have to rape them but they can get pregnant from it.

I haven't seen a censorship on the game yet, probably because my char is virgin
and I don't know whats DLC about and when it comes to Grappling when I press R my char just bend native over and rapes

DLC is about loli and shota content. Download it if you want to, or just because you want the maddest island experience.
It is impractical to use grapple when there are other enemies around. You need to do it with the last remaining enemy, or lure the target away from the group. Which makes sense - why should they just stand around and wait while you're raping their comrade?when it comes to Grappling when I press R my char just bend native over and rapes
Are you, perhaps, using grapple on knocked-out enemies? In this case, you should know that you can use it on conscious enemies too, male MC will hit their head against the floor or something while you click a lot. Which is how this ability substitutes for a faint weapon such as a club.
So, apparently steam is getting beaten up by Credit/debit cards with their "new" policy so Mad island might get delisted guys.....

You can get them from chests in the dungeon or by trading fighter orbs with the trader.

some people want the guide to be detail, some people only want the answer without understanding it and lazy to read. So, i chose the detail way because they will definitely ask more details of it

You need to do history quests and fight the fat friend that falls from the boat in the intro then when you defeat him you get some recipes from a chest one them is the bed
See that chest icon on the bottom-left? It hosts your collection of key items. Some of them allow you to make stuff you couldn't make before.
How to get key items? By adventuring, of course!
Oh, and in case you missed it, interacting with a crafting station changes the craftable list of items.
