Basically, there are two routes for each girl, a love route and a lust/NTR route. The love route choices are indicated by heart icons; if you get a choice with no hearts, it doesn't affect the route, and if you pick even one choice that isn't the heart icon, you get locked on the lust/NTR route. The best way to go about things, if you want to see all the content, is to play the love route first to completion, saving at each choice; at the end of the love route you'll have a scene with the girl. If you want to see the lust/NTR scene instead, go back to the last choice and pick the non-heart option.
The love route for each act 1 girl (Ursula/Ada/Rachel) unlocks a specific character for act 2 (Alana/Miho/Powell, I think), so you need to do at least one love route to just get to act 2 (if you do all lust/NTR for act 1 girls, you get an early game over). Same idea applies for act 2, play through story on love route to get those scenes, then go back to last choice before the scene and pick the non-heart option to see the lust/NTR scene (if you want). Do that for act 3 as well and you've done pretty much the entire game.
There is one character (Lana) who has a route that begins in the arcade rather than at the school (act 1) or restaurant (act 2); you'll start it by cheating at the arcade game. There's also some other characters that have scenes and/or small routes (Felicity has a small route in act 2, Gloria has a small route you can do if you get the TV from Ada's love route, Powell's daughter Clarice has some scenes, Albert has a scene, there's a homeless girl Nikki that you run into in the city/around town that eventually leads somewhere).
That should cover all the routes and pretty much all the scenes; specific endings or scenes you might not unlock unless you do specific things, but you can probably figure that out through playing, and if not, feel free to ask later on.
That's because you can have sex with a male character when the MC is a male too. You can see a picture of the scene in the samples at the first post.

I think you can unlock Clarice scenes if you play the love route of powell and you spank the ass of Clarice at the end.
Later, at some point in the last act I think, you will find Albert and Clarice ready to go to play some Dungeons and Dragons, you need to go there.
In the game, you need to choose between some squares to unlock a NTR or a love scene with Clarice.

So, I've been using URM on this game, and I got to the start of Miho's route, and I saw it called Rachel_True_Love, and apparently, it's False for me, which couldn't be right since I was doing all Love Routes, and then going back and doing the NTR Routes. However, when I looked using URM, I saw that there's actually a true_love and real_true_love for all 3 main girls. For me, all 3 true_love were False, and all 3 real_true_love are True. Is this gonna be an issue? Especially since Powell has true on both true_love and real_true_love...
Let me be honest with you and let me tell you a little story...
I started Night Lessons trying to make a monthly reward for my Patreon, the idea was to make a way to add girls and short stories, 2 pictures per girl. Then, after the first release, people in my Patreon like so much the girls, and they were asking about continue the story, so I did. After that, I spent around 2 years to finish the whole game.
In that time, I re-use a lot of names because I thought about finish the game at some many points (that's why you can see true_love, real_love, end_love and variables like that for each girl). So, I don't even know where the true_love, real_Love or any other variable goes, because I focused only to make the game works as much as I can.
Until I know, in this version, the most part of the story should be unlockable without problem if you follow the hearts for the loves routes and if you check the places before the end of each act.
Some parts of the story could be a little tricky, like Gloria or Niki ones or the Clarice D&D game, but the rest should be playable without problem.
Also, I don't know how you are using URM, but you need to count with the position of variables looking at the code, because some of them could make others impossible to get, at least what I'm understanding.
I guess I can't help so much aside this.

Tried to save Ursula at the concert but it wouldn't let me? The two choices had the same exact effect. Is this because I was sort of rude to her one time early on? Kind of annoying, guess I'll start from the beginning lol. (and by "kind of rude" i mean i said i didn't like to talk about myself...)

how do you get Lana to appear at the arcade again once you've tried the computer without a password a few times? i'm like 99% sure i went full love route with her. tried a bunch by messing around at the arcade in act 1 and now i'm in act 2 (or whatever the "remembering" act is) and i still can't find her anywhere.

another thing: the scenes with Gloria and Nikki seem to appear out of order or skip scenes - they refer to stuff that hasn't happened. the first time I met Nikki she went on about something we already talked about. with Gloria I saw her get felt up on the subway, and then the second time, she approached me as if I'd helped her with something, references to a gangbang and seduction, etc. (then the twist reveal)
If you played some old versions of NL, you should remove them before starts again, if that's not the case, the problem with Nikki and Gloria could be from the game, but I have no idea of how to repair it.
Right now NL is not a monetizable project anymore, and I'm working in another games, so I have no time to revise it, but I guess I'll do it at some point in the future, because I'm thinking of re-doing it after make the second part to try again in Steam, avoiding the "problematic content".
About Ursula, you have some little hearts in the options, if you miss one of them you fall in the NTR route.
The best way to play is choosing those, saving before a hentai scene and re-visiting just if you care about the NTR route.

just asking i now off topic but the powell game right. mybe after they finish there game they will see what they can do here.
but do not forget they need some down time and family time both of those are more inpotion thin these games.
