Well im not expect much from second game from that dev, the only thing actually i wanted is belly bulge from inflation, and better liquids inside cuz this game has x-ray, and also would be great to see stomach x-ray view.

There is two options in da game. Biological Father, and Step Father.
Google translate second option like Father in Law so idk actually Step Father it is or Father in law

If you're having the issue where the translations don't work without doing a reload, try this plugin (put the dll in BepInEx\Plugins). There may be a slight delay before they update, but it should solve the issue.
