Well they basiclly did this like in the bigger breasts and nude passcode. Add a hidden feature in the game that non-patreons are able to find, yet unable to decypher. Then the dev just describes the guide and passcode to unlock this feature.

It litterally says that in the pateron page. Also I got this info by looking into the archieved pateron stuff that is submitted by others.
