Identify Open world game with customization

looking for a game that had a lot of customization being bale to enable/disable a lot of fetishes including beast people and pregnancy, it was 2d i believe with the main character waking up on the top floor of a building? (not roof) and had a battling system to explore the world. some of the details might not be accurate but its coming from my vague memory. any help is appreciated
