I didn't really look into it, but Whoreizon seems to have a character creation. No idea what gameplay is like though.

For the most part that's gonna be a no. Most of the adult games are being developed independently or in very small teams of 2 or 3 people. A game on that scale is gonna take quite a few people and a lot of time and money.

well not many ... there are some unity and unreal games... but you will find they are somewhat laking.... u can howerver plan qsp games ... these are menu based games... i.e keyboard and links with IRL pics but there is some sememblence of rpg and openworld

As far as I know there are no real open world porn games or porn games with an actual character creation. The closest thing I played up to now is Sims 4 with Wicked Whims and some other mods for sexy lingerie, bdsm equipment and realistic genitals.
