ORS Chapter 12.3 FULL was released September 11th -- but added nothing but bug fixes to 12.3 BETA (which was released July 31th). So: 3 months?

The 12.3 FULL added the epilogue and bug fixes to the previous beta. The release cycle for new updates only begins after the FULL has been released because before that work is not being done on those new updates but on bug fixes for the previous update.
Saying updates took too long between the beta and FULL for chapter 12.3 would be a totally different complaint than saying the chapter 13.1 update took too long to release. The former probably being true.

In theory, but in practice sometimes new scenes are added in the final version and some others are edited, that can lead to new bugs

Yeah, I've already posted before how Ian could look like at the end of the game/his future older self(AI version).
I'm sure you already know this, but there is a variable for an Ian tattoo and beard. So there is always a possibility, just a matter of if EK wants to implement it. Considering the amount of extra effort it seems to add for Lena to have tattoos, I'm guessing if Ian and Holly get them then it will be pretty late in the games lifespan.
