How do you unlock Rachel and Grace and the caves area? I've run out of things to do on this playthrough and I think I missed a trigger. Olga also won't move off the ship.

I just played it a few weeks back, but your question is a little bit too complex to answer, the game is very very complex.
I just copied all the walkthroughs from all the girls in different word documents, so I could strikethrough the stuff I already had done.
Sometimes you need to finish a quest for one girl, to be able to progress with another.
On the left side you have a 'tab' were you can click on, where you see al the open quests, those quests you often can find in the walkthroughs, to see what needs to be done. Unfortunately you cannot see which quests have been done.
What I often found helped a lot is looking at your phone, and contacts, there you see the progress of each girl, most quests trigger when you have enough points with one girl or the other. So if you don't have 25, 50, 75, 100 points, you can try talking to them, if you answer the right questions they get points until the limit is reached, mostly that will trigger another quests.
Most you probably already knew, but it's the only hint I can give you, without replaying the whole game again...
