Such races are called "humanoid". There are no pigs here, etc. This is almost the same thing as orcs, elves, etc. Fictional, fantasy races. Ai ai...

I, like others, was struggling to get passed the orc. I was surprised when you said "at the beginning of the cave under the rocks" because I had gotten a game over just by walking there and never went back.
So I went back, and found the pad. The game made a comment about how maybe the orc will be distracted by it, so I went to try bypass the orc, and it didn't work. So I went to Martha's piss like you said, and got an instant game over again.
This was weird so I experimented a bit. There are 3 paths blocked by the Orc, right? I'll call them left, center & right. Going to either the left or center path, then going to the piss, causes an instant game over. Going straight from the pad to the piss works, and going from the right path orc path to the piss works. Going to the center Orc path, _then_ the right orc path, _then_ the piss also resulted in a game over.
Not sure what the exact bug is here, but something is buggy and is likely to explain some players frustration here.
Edit: After getting the piss-pad, going to the center path, then going to the shiny spot on the right path resulted in an instant game over as well. So to other players, I guess always use the right path when approaching the Orc.

Right now, I tried to find the bugs from your description and found nothing... The game runs normally under any conditions. But it's early in the morning now (we haven't slept yet) so I'll let the team know about your test and of course we'll fix it.
For now, play like this:
We go to the orc (right path), run away (cut-scene), return to the entrance to the cave and take Martha's pad.
We go to the urine, and after that we again go to the orc in the same way. The middle and left paths need to be repaired.

No, because anything you do will kill you, i had already understood the pissed pad trick, but it kills you, there are no way other then die. Try to fix that bug, it's frustrating.
Of course we will fix it. But for now try this:
Go to the orc (right path), run away (cut-scene), return to the entrance to the cave and take Martha's pad.
After it go to the urine, and after that again go to the orc in the same way (right). The middle and left paths need to be repaired.
