Well, you could just design an NPC quest with a perk that activates altering the looks for staff (surgery+) and spare the players the long walk for changing the looks'n'assets.
If you decide to implement a girl/boy friend, it comes down to maybe setting a "useful" perk and altering it to another "bad" one, when you decide to move on. Such a hefty bad perk, you better fire your ex-lover... Something like reporting drug use to cops, drastically altering the mood of contracted girls, the real deal stuff... So if transform your company to a cesspool of ex-lovers you should be struggling to just get a day without bad events. Also ex-lovers should not be able to shoot scenes together or with your actual lover (yeah, the bad trip)
But it may be not worth it, because from the tycoon point of view, what is the return of investment, what benefits can be gained?
If you make a relationship possible, please allow before that I can transition an actor/actress to staff then to PA then to relation, because I'd like to have a jealousy implemented for relations, which drops for 1% (difficulty level? Player perk?) for each scene shot. Meaning PC shoots a scene (or has sexual events/encounters), relationship target gets the jealousy substracted from happiness - so you won't be able hold a non-porn-queen for a long time while growing your company. So you have to "balance" your adventures vs relationship, or you have an ex-lover. After you "retire" and have your yacht... well... why retire at all?^^
