I cheated my stats up a bit (really just popped my level up an extra hundred since the stats that matter scale with it) and I was still getting my ass handed to me by some girls. I'm not really sure how you increase your life count in the actual sex part; I managed to get an extra one once and never found out how to do it again. MTL would probably help you learn how to play but I don't usually trust them since they look awful most of the time unless someone with good Japanese knowledge proofreads it all.

Your goal is to find the "G-Spot" in the x-ray at the corner of your screen. You need to move the penis up or down in order to find the right position to hit it. Usually, the G-Spot is located at the end (or near it) of the depth you've reached. Sometimes, however, you need to back up to find it (ex. all life bars have a G-Spot at the entrance even though you can go fully in).
Pressing 1, 2, or 3 on your keyboard (or scrolling with the mousewheel) will change sex speed, obviously by the number you press. Good for when you find the G-Spot and want to hurry up and get to the next life bar.
TAB and ESC do the same thing: bring up your stats, and a pause menu. Second button in the list is "SAVE". Use it often (obviously). Third is Options (Literally just volume AFAIK). Fourth exits game.
Pressing Z interacts with things. This is a universal interact button that basically works for anything you have to interact with: Girls, shop menus, or equipment.

They give you stat bonuses, but sometimes there's a catch; usually lowering another stat in exchange for a boost to one of your stats. You can hold 5 at a time. Try to pick up another and you'll be asked to either toss it or make space by tossing out one of the items you have already.
Successful sex with girls will give you EXP and Cash. You can use cash to buy Cat Food and the Flashlight in the shop two doors down, and semen refills and a mysterious key from the lady across from it with her tits hanging out.
Sex with girls will also lower your semen count, whether you win or fail. This is the white bar at the bottom of your screen, under your level and EXP. If it completely drains, you will be forced to skip the rest of the day, heading straight to night time.
