Now, jokes aside, might be a GPU bug, check your drivers. Or could be windows update completely crunching overloading your ssd or hdd for breakfast, as these sometimes might cause graphical glitches (since the game can't load the model in time), make sure to not game while windows updates itself(y). If your not using windows then I don't know, could be something else.
