Identify Solved - Can't remember game

there was what i believe was a renpy game which i think was a remake

i believe it starts off with in a sandbox like game where you go to a café and then a hospital
you are a doctor diagnosing patients through there symptoms and go do it all over again,
it loops a few time with things changing

i don't believe this the main game but a prologue

neither
my memory isn't the best but i think it around last year
i believe it was using daz and the game was on renpy

i remember
you wake up (1 image of your room)
you then travel to a cafe (clicking circles navigating through a city, the view were mid/high level shots inside a city)
you can then go to a cafe and talk with the worker
when in the hospital you sit at desk, read 1 line of symptoms and diagnose them from three options, i don't think they really mattered. you did this around three time before you wake up the next day and do it again.

wasnt much as i think it was setting up for the main story
