Alright lets cut to the chase.
Instead of being stranded on an island with however many women you decide to mod in (before your CPU sets on fire -- thx unity), instead you come into ownership of a resort/hotel/hot springs/sex hotel/whatever. Depending on difficulty setting (soft, half-chub, hard), the starting building can be one of three tiers (struggling, new and established). End-game is essentially getting all the swanky expensive stuff to bling out your resort to however you see fit and the reputation that comes with running a 5-star sex-spot (or whatever.. more on that later).
Rather basic, but this is where the card-sharing comes into play; stealing a page from Dark Souls (then drawing crayon all over it), you as the player can visit other resorts and enjoy their services! You can even tag cards to either be workers at your resort (grounds keeper, maid, receptionist, etc), "customers" that use your resort and can also "travel" (card sharing), or both! I suppose if you wanna go the full 100 yards (or 91.44 meters for the weirdos) for the score, you could also enable "guest" reviews where guests can leave a review of your place and rate it depending on their given values. And yes, you can also rate someone else's guests too.
Of course, there will be a myriad of options to be able to filter out certain guests to cater to everybody's tastes and preferences.
Now to start off with already existing assets, the ruined island could be a functional community that serves as a single-player functional add-on. It has shops and other places to stock up on supplies. Maybe a fisher 'n farming family. But really, half dozen NPCs at most to keep CPU load to minimum? Retcon existing "high tech" infrastructure to something about company science output being shutdown and community needing the resort to replace the lost income? I suppose eventually you could choose between different areas to build your resort; ski-resort anyone?
From a business angle your resort/hotel/whatever could offer additional activities that you could either lead or let one of your workers run (so you can join in and/or take screen shots!); a "fishing trip" on a boat/yatch, scuba diving, a hike through the island to a resort spot (a pool or shrine or something)?
I suppose one way to share clothing cards and building cards would be to have a clothing store and general store that gives you the option to look at the newest trends and fashions through some new-fangled technology called The Internets.
Anyways, onto Reputation! So, uh.. long story short Reputation would replace the standard story progression. But it also comes in two parts; global and local reputation. Local reputation is how the local town views you; they like you they'll do you favors (ala shan) and possibly give discounts (and would enable you to import more expensive and larger goods). Global reputation would effect smaller things like worker morale, ability to attract guests outside of preferred destination, and most likely your personnel ego.
Now, how to strike a balance between resort management sim and sex game? Individual options and cheats of course. Point is to have fun! So you could literally just hire someone to run the entire resort itself (auto-pilot, no punishment or anything) and focus on banging all the sexy individuals that stay at your place, whatever it may be. Having a bunch of pre-made buildings of different variety for those un-interested in building to start off with is also a must. Wanna focus on running the business? Go ham, then take a vacation!
