Question for the people that know. How do decensor mods work? I decompiled the source files, and all the source images themselves are censored, so it's not something done at runtime. Are people going in and modifying the source images to recreate the uncensored images and then replacing the original censored ones?

I've done that. Walked the dog, shared with the homies(confessional), banged in front of jesus(aft), banged in front of jesus(night), then climax at all positions. Nothing happened. Dunno if it's a bug or something I'm not doing. it's still at 3/4
You can go to the church on even-numbered days, talk to Rika in the afternoon, and then choose to have sex in the church. After that, you can see the NPC group interaction, and then Rika will also complete her fall.

When walking the dog, did you wait until the end after climaxing three times and go outside and see those events? That was the problem for someone else.
Just put the mouse on the hearts to have hints. Be careful that 119 chaos is quite high and you risk having a game over. Over 100 is fine to have all events, but not that much. At least for now, no idea if in the future they give option to increase chaos, to see the news reporter naked for example.

I had the same issue, you do the dog walking until it progresses to dog walking outside then sing again
