It's one of those things, so many people want it, we'd be idiots not to include it at some point or another. But it's also a low priority, especially with chapter 2 on the horizon. And, as Shiny and I think Ron have both said in the past, it'd probably require an entirely new set of systems to be added in, it's not just a matter of 'pres buttan, reciev ballgag'.
As always, it's rarely a matter of 'we don't want to', more often than not, it's a case of 'we can't right now'.

well as a nice side-thing a ballgag could be added as a static item to be randomly left in the room / bathroom after some action.... that would be fairly easier to do and could provide for some good dialogue source material.
