It's from the universal decensor, go to BepInEx\plugins and get rid of the DumbRendererDemosaic.dll file. You won't get that weird x-ray effect with the blowjob but the top mosaic...

No a full decensor would mean editing the actual artwork as there's two layers of mosaic. The full game is here with second mosaic removed, but it will mess up the blowjob.
