I dont remember the name or tags. I just remember that: In the world, women were the stronger sex. Its a Male Protag, who has a huge schlong, think Feminine looking. And the prologue is in a hospital, isekai like I think? You bang the nurse or doctor before being let free into the world.

Twisted World, though you forgot that before the hospital you spend a little time in heaven & hell. Also be aware dev started a remake, so either play older builds or wait a while for updates to get more content.
