Summertime Saga has a pregnancy mechanic and a few characters that are less than thrilled about your decision to knock them up, but at the moment that's a complex as it gets.
Dr. Amana has a pregnancy mechanic as well and the creator promises to use it as a drama generator. I think this one is more likely to be what you're looking for, but it isn't quite there yet.
