I know what the patch is for but what does it actually change in the game itself? Do you lose actual scenes, characters etc or does it just delate all family relationship references?

An incest patch is used to restore family relationships in games, as they were mostly removed due to Patreon policies (Incest is not allowed).
It usually only changes the label of some characters.
e.g. landlady -> mother
In some cases the text is adjusted, but rarely new content is added.
