For me, it's definitely the forbidden aspect. What I really think it boils down to, and this applies to many various kinks/fetishes, is the concept that it's this forbidden/taboo thing, that more or less increases the erotic fervor for some folks just because it is taboo... Not really that it necessarily means the person/s fantasize about doing it in real life... It's just this outside looking in taboo fetish, that some people feels heightens the erotic intensity... And some humans seem to get their rocks off on things normally considered taboo/forbidden, just because they are taboo/forbidden in a fictional setting...

Now that being said, there is a very tiny portion of folks out there that probably fantasize about doing it in real life, but they are the exception and not the rule...

Also, there was a moment several years back where a very few VN/Games using that theme began making all sorts of money on sites like Patreon and so forth... Which increased the hype for new developers wanting to get a piece of that pie themselves... Causing a very intense bloom in the use of that very specific fetish, to include a high use of the exact same game play devices to go along with it, in many cases... To this day we are still seeing the outcome of that bloom, but it is dying down very slightly compared to a few years back... When in fact it is more popular for Developers then for the general populace of readers/players in this niche market... Because it lacks originality and gets over used by developers, especially newer developers...

Can you really turned on by a story that is abhorrent to you as a person? I mean of course there is aesthetics, and there is story. For me incest disturbs me from enjoying the rest of the story. I admit I'm quite inflexible in this manner, the same way as I avoid stories with trans storylines. I'm an accepting person, let thousand flowers bloom, I'm even OK with consensual incest (while I think reproduction might not be a good idea) but I just cannot relate with these plots personally.

It's a fetish/kink that's very easy to accommodate in a game, has a very enthusiastic fanbase(many if not most of the top adult games on Patreon have an incest theme as primary/central to the game), hits the general "taboo/forbidden sex" button effectively, and provides a real obstacle that means the sex isn't going to happen right away. I stand by my opinion though. It seems like a standard go-to. It lacks nuance, you know how it ends. As a game designer I feel you and I get it that you are offended... But a great, unique story needs suspense, mystery, uncertainty of outcome, choices that matter... not cheap cliches like incest.

The best stories place you in the skin of the protagonist. One feels conflicted when one is supposed to relate to something, he normally wouldn't. Of course sometimes intellectual experience may come to that, like in "death is my trade" by Merle, where you can relate to the SS officer understanding how he drifted to the place he ended up. At the same time you are conflicted as all decent people should because the things he did, regardless of the vents led to it.
The same things can not be said about incest... If you have a boner, or enjoy the story, it is hard to understand how you not relate to it... It is the same thing like with the last of us 2... you cannot relate to characters so horrible emotionally.
If you keep out the context, incest stories are quite boring. If you embrace the thrill of fucking a close relative... well then our minds works totally different. If you are turned on by a gay scene, then you must be at least a bit interested in the same sex whether you admit or not. There is nothing wrong with this, more power to you, but I believe - and I'm of course no authority in the matter - with incest it should work similarly.

If someone enjoys playing a game where the main character enjoys seducing nuns, does it logically follow that the player must secretly desire to seduce nuns IRL? Identification with the main character doesn't necessarily involve agreement with their goals and motivations, it means some basic understanding of those goals and motivations. Trevor in GTA V is absolutely reprehensible, but can be entertaining to play, the same way one might enjoy playing the robbers in "cops and robbers".

There's an allure of the taboo/titillation of the forbidden fruit. Combined with the challenge of achieving something that logically should be extremely difficult if not impossible to pull off, this has a more general appeal to the "thrillseeking" and "boundary crossing" parts of the brain. You don't have to secretly want to fuck your family to enjoy incest porn/games.
