Netorare isn't just "cheating." It's when an object of affection gets emotionally stolen from the "sympathetic" character. For NTR to happen, the character doesn't need a formal relationship with the girl, just a loving/affectionate connection. This is why there can be NTR stories with mom, sister, childhood friend, and whoever you can think of because it's not about cheating. It's about corrupting or destroying the affection between the "good" guy and his loved ones and inciting jealousy (In the player/reader or the character). Also, considering that many porn games have incest, it wouldn't be weird to assume that an MC could have incestuous feelings that would make it feel like someone is stealing his girl.

I hope that our MC will bring John to clean water for everyone. It's a pity if his mother becomes his prey, throughout the game you become attached to her and as you progress you understand all the rottenness of John and worry about the girls.
