It's a random drop from one of the Orcs in the top left of the map. You don't need it - just attempt to talk to the woman in the next bed in the slave quarters. She will signal 'A1' to you. This means (obviously :-/) go upstairs in the pub and knock on the leftmost door.

One thing that could be addad that I feel would fit in the game is a random event that when Vicky is walking the town at night, there is a random chance of a rape scene. She is in a town of horny perverts after all.

Also the bug that chrashes the game "ysp reference is not defined " when the blowjob scene at the tax man and the glory hole is a bit annoying.
