do you see that blue bar under the hearts? you need to fill that one,the 99% that you see its to get purple hearts and you cant get a purple heart if you dont have any other red heart

For red hearts, which unlock the ability to upgrade them to purple, you need to fill not the blue "alertness/awakening" bar, but the red "sympathy" bar...
