No, it means he is male, there is no additional content aside from being able to transform to female , but you can do visa vera now if your start as a woman

You have to have charmed Anita. Send her to look after him, then she and Bambi move him to the basement.

Talk to Mayor Thomas. You will receive a message from Angela when the option is available, but it is later in the game once you control most of town hall and the police

It is not a mod you apply to the base game. It is a standalone copy of the game made at version 14.8.4, it is not compatible with the mod system that was later added to the base game.

To play the mod, just download and play and do nothing with the base game or any updates. You will get the content bologna44 added to the game, but miss out on content added to the game since version 14.8.4
