The diary entry is a separate thing off to the side. And reading it didn't alter the ending for me. But there's nothing wrong with staying on the island. You get a happy pregnancy ending from it and the monkeys do nothing to you.

Only when you pick up and keep the item I mentioned before. Does the island ending get altered. Ending with you getting cucked and the mother getting gangbanged.
