I haven’t played the game yet. Is it possible to change the relationship in the game from aunt/nephew to mother/son by adjusting the patch or intro and related settings? Or would that affect the story?

Fixed in v0.3b. If you wish to continue with v0.3a, you can turn off alert notifications in the preferences menu, or wait a few days for v0.3b to be released publicly.

Wait, this is abanonded? That's news to me. And hasn't Patreon been banning pages with even patched incest lately (even 3rd party I've heard)? I know they pretty much digitally stalk a dev all over the internet to enforce that (which should be illegal).

This game got so much potential if it was playable like any other game out there. Like, bro dev, all you got to do make sure your game can be play without weird shit happening. You supposed to fulfill consumer demand not yours unless you're making the game for your own fap. Btw, you're on pirate site, should expect pirating happened. Those patreon number can be jacked up if you're actually deliver.

It would absolutely affect the story. There decent chunk of dialogue towards the beginning with your aunt about your mother, her wife and your sisters. And based on how they were talked about I would imagine that at some point they will be introduced to story.
You can't complain about "consumer demand" whilst pirating shit at the same time, buddy.
eh after how many months we get an update ? and its just to keep the game alive ?

Microsoft make billions from letting their user pirating. Something you should learn as a startup. If your product make people felt there's no need to get pirated, you will eventually rewarded. Unless you're trying to be Adobe. Watch a few game thread on this site, you will realize those dev got jacked up in number since their game got posted on this pirate site. At the same time, trying to improve their game by fulfilling some demand from this site.
