I understand the spirit behind the survey, but I think you might be giving short shrift to the idea that a character is more than the sum of their fan services. Maybe that perspective makes my responses even less worthwhile than they'd otherwise be.
1) 99% pleasure. The 1% pain is to make pretense to realism, not because it's hot in and of itself. It can be difficult to draw a clear line between feeling pain and experiencing intensity. If you get rid of all of that just to make sure there's never the merest suggestion of pain, you might lose something.
2) It depends on the character, but overall, I'd say a genuine, informed preference is hotter.
3) Ass-to-mouth is one of the reasons God invented porn. Iz no smellz, iz no tastes, iz no infectionz. If it's just porn and not real life, awesome. Go for it. Go absolutely crazy.
4) They've all got their charms if done well.
5) Tight. It's fiction. Might as well pander to the fantasy of "awesome immortality" -- infinite mileage with zero wear and tear.
6) I don't go out of my way for it, but it can be done well. No strong preference. I'm super into girl-on-girl stuff, though, so I wouldn't love a vagina-having character who was 100% opposed to any vagina/clit action whatsoever. If it's just a concern about a literal penis in the vagina, then I have no strong feelings about it.
7) No preference. Make it work with the rest of the character, and either can be hot. I have a weak spot for more introspective and reserved partners who turn out to love sex (whether they already know or come to the realization,) but that doesn't mean I can't appreciate a confident, experienced, sex-positive one.
